What you need!!
1. Team roster filled out on this web site. Click on team name below:
a. Click Team Log In:
1. To get pin #, hit click here
2. Enter your email address, hit submit. A pin # will be emailed to you.
b. When you get pin #, re-enter, log in and do the following:
c. Click Roster
d. Click Create Multiple Players
e. Select # of Players
f. Enter for each player:
1. Jersey #
2. Position
3. First name
4. Last Name
5. Player pass number under ID.
6. Save
This must be completed prior to midnight Wed. 8/29/07, so that we can have game reports prepared. Thanks for your cooperation.
Bring to Check-in the following:
1. State Certified Roster
2. Player Passes
3. Copies of Medical Releases(notarized) Copy under Puma Cup Tournament Forms
4. Contact Form: Copy of under 
Puma Cup Tournament Forms
5. Guest Player Form(State form, signed by other club)
6. Out of State Travel Form(only if out of State team)
